Schedule and Guidelines of Annual Medical Examination for A.Y. 2023-2024

As part of the annual requirement for enrollment, students are required to submit laboratory tests to the College Clinic and undergo Medical Examination.

Below is the process in obtaining a Medical Certificate:

  1. Present the request form to the medical laboratory of your choice.

๐—ก๐—ผ๐˜๐—ฒ: The request form will be provided by your program coordinator.

  1. Once you have the laboratory results, second-year to fourth-year students must book an appointment thru this link: bit.ly/GCMCUpperClass

๐—ก๐—ผ๐˜๐—ฒ: for incoming freshmen, the link will be provided by their respective coordinator after their interview.

  1. The Health Services Unit will confirm the appointment date thru email. ๐—ข๐—ป๐—น๐˜† ๐˜€๐˜๐˜‚๐—ฑ๐—ฒ๐—ป๐˜๐˜€ with an appointment will be entertained on the ๐˜€๐—ฐ๐—ต๐—ฒ๐—ฑ๐˜‚๐—น๐—ฒ๐—ฑ ๐—ฑ๐—ฎ๐˜๐—ฒ.
  2. On the Physical Examination day, bring the following:
    โœ…All the original results of your laboratory tests (an updated result or at least 2 months period);
    โœ…1ร—1 picture; and
    โœ…vaccine card

๐—ก๐—ผ๐˜๐—ฒ: for working students who had their physical done for the past 2 months, a certified true copy of laboratory and x-rays are required.
