• Filipino Citizen
• Physically and Mentally Fit
• Accepted to Gordon College Midwifery Program
• Has no existing scholarship grants
• Priority Applicants are;
  • Indigenous People or Member of a Minority Group
  • Dependents of Barangay Workers
  • Dependents of Government Employees
  • Victim of Calamities

SCHOLARSHIP PACKAGE

• PSA Birth Certificate
• Certificate of Good Moral Character issued by Barangay or School
• Medical Certificate that the applicant is Mentally and Physically Fit issued by a Government Physician (with photocopy of laboratory results including HBsAG Screening Test)
• Affidavit of no existing scholarship grants
• Certificate of Acceptance in Gordon College Midwifery Program
• BIR Annual Filing of Tax of both Parents/ Guardian or BIR Tax Exemption Certificate
• For Priority Applicants
  • People (NCIP) Certificate (for IPs or Member of a Minority Group)
  • Certificate of Employment (for Dependents)
  • Certificate from Local DRRMO or DSWD (for victims of calamities)
