Step 1. Book an appointment for in-person photo and signature capture processing through

Step 2. Proceed to Rm. 415 on your appointment schedule. Kindly follow the exact schedule in your appointment (not later or earlier, to avoid any inconvenience).
Read and understand the General Reminders and the Data Privacy Notice for ID Processing for the Academic Year 2026-2027.
NOTE: If you cannot make it to your appointment schedule, due to personal reasons, class suspension, administrative rescheduling or holiday, you need to CREATE A NEW APPOINTMENT.
GENERAL REMINDERS:
- For those who will be applying for ID replacement, you need to pay the ID replacement fee of 200.00php. You must bring the receipt of your payment on the day of your appointment.
- For lost ID cards, affidavit of loss is required. For ID replacement due to update/alteration of information requires your old GC ID card.
- For change of address, name or program, this must be approved by the College of Registrar.
- To all students that wanted to update their photo, wear your school uniform. For working students with uniform exemption pass, kindly wear polo, polo-shirt or any collared top.
- Do not wear a head covering that obscures the hair or hairline, unless worn daily for a religious purpose.
- Headphones, wireless hands-free devices, or similar items are not allowed.
- Eyeglasses are not allowed, except in rare circumstances when eyeglasses cannot be removed for medical reasons, e.g., recent ocular surgery and the eyeglasses are necessary to protect the applicant’s eyes.
- The frames of the eyeglasses must not cover the eyes.
- There must not be glare on eyeglasses that obscures the eyes.
- If you normally wear a hearing device or similar articles, they may be worn in your photo.
NO APPOINTMENT, NO TRANSACTION will be implemented.
