As part of the annual requirement for enrollment, students are required to submit laboratory tests to the College Clinic and undergo Medical Examination.<\/p>\n\n\n\n

Below is the process in obtaining a Medical Certificate:<\/p>\n\n\n\n

  1. Present the request form to the medical laboratory of your choice.<\/li>\n<\/ol>\n\n\n\n

    \ud835\udde1\ud835\uddfc\ud835\ude01\ud835\uddf2: The request form will be provided by your program coordinator.<\/p>\n\n\n\n

    1. Once you have the laboratory results, second-year to fourth-year students must book an appointment thru this link: bit.ly\/GCMCUpperClass<\/a><\/li>\n<\/ol>\n\n\n\n

      \ud835\udde1\ud835\uddfc\ud835\ude01\ud835\uddf2: for incoming freshmen, the link will be provided by their respective coordinator after their interview.<\/p>\n\n\n\n

      1. The Health Services Unit will confirm the appointment date thru email. \ud835\udde2\ud835\uddfb\ud835\uddf9\ud835\ude06 \ud835\ude00\ud835\ude01\ud835\ude02\ud835\uddf1\ud835\uddf2\ud835\uddfb\ud835\ude01\ud835\ude00 with an appointment will be entertained on the \ud835\ude00\ud835\uddf0\ud835\uddf5\ud835\uddf2\ud835\uddf1\ud835\ude02\ud835\uddf9\ud835\uddf2\ud835\uddf1 \ud835\uddf1\ud835\uddee\ud835\ude01\ud835\uddf2.<\/li>\n\n\n\n
      2. On the Physical Examination day, bring the following:
        \u2705All the original results of your laboratory tests (an updated result or at least 2 months period);
        \u27051\u00d71 picture; and
        \u2705vaccine card<\/li>\n<\/ol>\n\n\n\n

        \ud835\udde1\ud835\uddfc\ud835\ude01\ud835\uddf2: for working students who had their physical done for the past 2 months, a certified true copy of laboratory and x-rays are required.<\/p>\n\n\n\n
